Diabetic retinopathy is one of the most common complications of diabetes, affecting the small blood vessels in the retina. Over time, high blood sugar can weaken these vessels, causing them to leak fluid or bleed, which can distort vision. While managing blood glucose, blood pressure, and cholesterol levels through medication and regular checkups remains the cornerstone of treatment, research suggests that certain foods can offer supportive, protective benefits for retinal health.
No single food is a cure, but dietitians point to three categories with solid evidence behind them—leafy greens, fatty fish, and deeply colored berries. These foods provide key nutrients that may help slow the progression of retinopathy by reducing oxidative stress, fighting inflammation, and improving circulation in the tiny vessels of the eye.
Leafy greens for blood vessel protection
Dark, leafy greens like spinach, kale, Swiss chard, and collard greens are rich in two carotenoids—lutein and zeaxanthin—that concentrate naturally in the macula, the central part of the retina. These compounds act like internal sunglasses, filtering harmful blue light and neutralizing free radicals that can damage retinal cells.
A 2023 review in Nutrients found that higher dietary intake of lutein and zeaxanthin was consistently associated with a lower risk of diabetic retinopathy and age-related macular degeneration. Dietitians recommend aiming for one to two servings of cooked or raw leafy greens daily. A simple lunch salad with baby spinach and chopped kale, or a side of sautéed Swiss chard at dinner, can provide a meaningful dose of these protective pigments.
“Think of lutein and zeaxanthin as sunscreen for your retina,” says registered dietitian Maya Ramos, RD. “They help filter out damaging wavelengths before they can trigger inflammatory cascades in already-compromised vessels.”
Fatty fish for omega-3 anti-inflammatory support
Cold-water fatty fish—salmon, mackerel, sardines, herring, and trout—are the best dietary sources of long-chain omega-3 fatty acids, specifically EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid). DHA is a structural component of retinal cell membranes, while EPA helps dampen chronic systemic inflammation that can worsen microvascular damage.
In a 2022 meta-analysis of cohort studies published in JAMA Ophthalmology, participants who consumed at least two servings of oily fish per week showed a 24% lower incidence of diabetic retinopathy compared to those who ate fish infrequently. The omega-3s appear to reduce the expression of vascular endothelial growth factor (VEGF), a protein that promotes abnormal blood vessel growth—a hallmark of advanced retinopathy.
Dietitians suggest integrating fatty fish into meals at least twice a week. Canned sardines or mackerel make an easy lunch option, while grilled salmon fillets can become a quick weeknight dinner. For those who do not eat fish, algae-based DHA supplements may offer some benefit, though whole food sources are preferable due to the synergistic effects of other nutrients.
Berries for oxidative stress reduction
Blueberries, blackberries, strawberries, and açai berries are exceptionally high in anthocyanins—flavonoid pigments that give them their deep purple, red, or blue hues. These compounds are potent antioxidants that help neutralize reactive oxygen species generated by chronic hyperglycemia. By reducing oxidative stress inside retinal cells, anthocyanins may help preserve the integrity of capillary walls and delay the onset of retinopathy.
A 2021 clinical trial in Antioxidants gave participants with type 2 diabetes either a freeze-dried blueberry powder (equivalent to about one cup of fresh berries) or a placebo for eight weeks. The berry group showed significant reductions in markers of oxidative damage and improved retinal microcirculation on imaging exams.
Dietitians emphasize that a half-cup to one cup of fresh or frozen berries per day is a realistic, low-sugar way to support eye health. Frozen berries retain most of their anthocyanin content, making them a year-round option. Adding them to oatmeal, yogurt, or smoothies is a practical habit—no special preparation needed.
How these foods work together
While each of these foods offers unique protective compounds, their effects are additive when eaten as part of an overall anti-inflammatory dietary pattern. The Mediterranean diet, rich in leafy greens, fatty fish, fruits, vegetables, olive oil, and legumes, has been repeatedly associated with lower rates of diabetic retinopathy progression.
What matters most, says dietitian Jacqueline Chen, RDN, is consistency. “Eating these foods once a month won't move the needle,” she notes. “But making them a regular part of your weekly eating pattern can help create a biochemical environment that's less hostile to retinal blood vessels.”
Beyond these specific foods, keeping overall carbohydrate intake moderate and prioritizing fiber-rich whole grains, legumes, and non-starchy vegetables can help stabilize blood sugar, which directly reduces the metabolic stress on retinal capillaries.
Caveats and reminders
Dietary changes are supportive, not substitutive. Anyone with diabetic retinopathy should continue working closely with an endocrinologist and an ophthalmologist to monitor their condition. Blood sugar control through medication (insulin or oral agents), blood pressure management, and regular dilated eye exams remain the standard of care.
Some of these foods contain vitamin K (leafy greens) or omega-3s (fish), which can interact with blood-thinning medications like warfarin. People on anticoagulants should discuss significant dietary changes with their healthcare provider before increasing intake of these foods.
Furthermore, for individuals with advanced kidney disease—a common complication of diabetes—high-potassium greens like Swiss chard or spinach may need to be limited. A dietitian familiar with the person's full medical history can provide personalized guidance.
