If you are in your first trimester, you have likely heard the term “prenatal testing” and maybe felt a tingle of anxiety. That is normal. The goal of these early screenings and tests is not to worry you — it is to give you clear information about your pregnancy so you can make confident decisions with your care team. Here is a straightforward walk-through of how prenatal testing works, why timing matters, and what each result can realistically tell you.
Why screen during the first trimester?
The first trimester — roughly week 10 through week 13 — is a window when several key markers become measurable. Your obstetrician or midwife will typically offer a combination of blood work and an ultrasound. Together, these tools assess the probability of certain chromosomal conditions, most commonly Down syndrome (trisomy 21), Edwards syndrome (trisomy 18), and Patau syndrome (trisomy 13). They also give a preliminary look at the baby’s anatomy and your own uterine health.
Think of first-trimester screening as a flagging system. It does not diagnose a problem; it tells you whether your “base odds” have shifted. A low-risk result is reassuring for many parents. A higher-risk result opens the door to more precise testing if you choose to pursue it.
What actually happens during the screening
There are two main parts to the standard first-trimester screen: a blood draw from you, and an ultrasound known as the nuchal translucency (NT) scan.
The blood test (serum screening)
Your blood is checked for two substances: pregnancy-associated plasma protein A (PAPP-A) and human chorionic gonadotropin (hCG). The levels of these proteins change meaningfully when a chromosomal condition is present. For example, low PAPP-A combined with high hCG can indicate an increased likelihood of Down syndrome. The lab takes about a week to return results.
The nuchal translucency ultrasound
At the same appointment, a sonographer measures the clear space at the back of your baby’s neck. Every baby has some fluid there; when the measurement is thicker than expected, it can signal a higher chance of a chromosomal issue or a heart defect. The NT scan is safe, non-invasive, and does not hurt — you just lie on the table with a full bladder for better image clarity.
Your doctor combines the blood results, the NT measurement, and your age into a calculation. The output is not a yes or no; it is a risk ratio, like “1 in 900” (low risk) or “1 in 50” (higher risk).
NIPT: a newer option many parents choose
You might also hear about non-invasive prenatal testing (NIPT), sometimes called cell-free DNA screening. This can be done as early as week 10. It analyzes tiny fragments of your baby’s DNA that are circulating in your bloodstream. NIPT is not a routine part of every practice yet, but it is widely available and often covered by insurance for certain age and risk criteria.
Compared to the standard first-trimester screen, NIPT has a higher detection rate for trisomies and a lower false-positive rate. That means fewer “scares” that turn out to be nothing. However, it is still a screening test — not a diagnosis. It does not catch every possible condition, and it cannot detect structural abnormalities like a cleft palate. Many parents who want maximum information without invasive risk choose NIPT first.
A quick note on accuracy: No screening test is 100%. A negative result is reassuring but does not guarantee a perfectly healthy baby. A positive result is a strong signal that something may be present — not a certainty.
Diagnostic tests: when screening results suggest a closer look
If your screening shows increased risk, or if you are over 35 and prefer definitive answers, your provider may offer diagnostic testing: chorionic villus sampling (CVS) or amniocentesis.
Chorionic villus sampling (CVS)
CVS is typically done between weeks 10 and 13. A doctor uses a thin needle or catheter to take a small sample of placental tissue. Because the placenta forms from the same fertilized egg as the baby, its chromosomes match. CVS can diagnose Down syndrome, trisomy 18, and many other genetic conditions. Results come within a week or two.
Risks: CVS carries a small risk of miscarriage — estimates range from 1 in 300 to 1 in 500 procedures, though recent studies with experienced doctors show numbers closer to 1 in 900. It does not detect neural tube defects such as spina bifida.
Amniocentesis
Amniocentesis is performed later, usually between weeks 15 and 20. A doctor removes a small amount of amniotic fluid with a needle guided by ultrasound. The fluid contains fetal cells that can be analyzed for chromosomal and genetic conditions. It also checks for neural tube defects — something CVS cannot do. Miscarriage risk from amniocentesis is roughly 1 in 500 to 1 in 900.

How to handle the results
No matter which test you choose, the waiting period is often the hardest part. If your results come back low-risk, take the reassurance and continue regular prenatal care. If they come back higher-risk or positive, you have decisions to make — but you do not have to make them alone. Genetic counselors are specifically trained to explain what the numbers mean, walk you through your options, and support you with emotional clarity, not pressure.
It is also wise to talk with your partner or a trusted person about your values beforehand. Would you continue the pregnancy regardless? Would a diagnosis change your birth plan or lead you to specialty pediatric care? There is no “right” answer — only what is right for your family. The testing is a tool; you hold the decision.
Finally, remember that most first-trimester screens turn out just fine. A high-risk result is scary, but the vast majority of those pregnancies — especially after age 35 — still result in a healthy baby. Testing gives you information, but it cannot predict everything. Lean on your care team, ask every question, and give yourself permission to feel whatever comes up.
