It’s a core feature for many—up to 80% of people with PCOS have some degree of insulin resistance, even if their blood sugar looks normal in a quick panel. The tricky part is that insulin resistance doesn’t always announce itself. It whispers. Fatigue after meals, stubborn weight gain around the midsection, skin tags, or dark, velvety patches on the neck or underarms (acanthosis nigricans) can all be signs.
Understanding what drives insulin resistance in your body is the first step to managing it. Below, we look at five common underlying causes—and more importantly, how you can track them in your daily life without feeling like you’re running a lab.
1. Chronic inflammation
Inflammation is your body’s natural alarm system—but when it stays switched on, it can interfere with how your cells respond to insulin. In PCOS, low-grade inflammation is common, partly because of hormonal imbalances and visceral fat. When inflammatory markers like C-reactive protein (CRP) are elevated, your cells become less responsive to insulin’s signal to take up glucose. The result? Your pancreas pumps out more and more insulin, leading to higher circulating levels.
How to track it: You don’t need a blood test every week. Notice how your body feels after certain foods (especially processed carbs, fried items, or added sugars). Some people find that joint stiffness, brain fog, or persistent skin breakouts correlate with inflammation. Over time, a food and symptom journal can reveal patterns. Keep an eye on markers like CRP and homocysteine with your doctor once or twice a year.
2. Excess visceral fat
Not all fat is created equal. Fat stored around your internal organs—visceral fat—is metabolically active and releases substances that make your cells more resistant to insulin. Many people with PCOS carry weight around the midsection, sometimes even at a “normal” BMI. That’s why measuring waist circumference matters more than the number on the scale.

It’s a two-way street: high insulin can increase androgen production, and high androgens can worsen insulin resistance. Breaking that cycle is essential.

4. Poor sleep and circadian disruption

If you snore loudly or wake up gasping, mention it to your doctor; a sleep study could be useful. Simple changes like a consistent wind-down routine and avoiding screens an hour before bed can make a surprising difference.
5. Nutrient shortfalls (magnesium, chromium, vitamin D)
Your cells need specific micronutrients for insulin to do its job. Magnesium helps regulate glucose transport; chromium enhances insulin’s binding to receptors; vitamin D influences insulin secretion and sensitivity. Many people with PCOS are deficient in one or more of these, often because of diet, absorption issues, or hormonal effects on nutrient metabolism.
How to track it: Ask your doctor for a blood test that includes magnesium (RBC magnesium is more accurate than serum), vitamin D (25-hydroxy), and a basic metabolic panel.
